Fire shines light on failures

The loader was being refuelled at a surface fuel bay when the fire started and the worker immediately shut off the fuel before the automatic fire suppression system kicked in.

He then tried to reach the fuel bay's fire hydrant however it was not accessible due to poor housekeeping.

The fire is being investigated by the mine owner.

It was also found that a 50kg fire extinguisher trolley nearby was overdue for inspection.

The NSW Resources Regulator said mine operators should routinely audit fire-fighting equipment to ensure it was maintained in a fit-for-purpose state.

That includes carrying out maintenance inspections and ensuring housekeeping around control devices did not hinder emergency responses.

The regulator said mine operators should also audit fire-fighting capabilities around refuelling areas to ensure they could manage any fire events that might occur.
