Quarry fatality warnings

AN INSUFFICIENT windrow and narrowing haul road contributed to a fatal haul truck incident at a quarry at Red Hill, east of Perth, Western Australia, a Department of Mines, Industry Regulation and Safety significant incident report has found.

According to the report the fatality occurred on June 20 when a 110t haul truck crossed a windrow and fell down a pit wall at the Hanson Construction Materials Red Hill site, killing its 40 year old operator....
