The cabin and front of the ute was flattened by the truck’s tyre, as seen in images which have emerged.

Fortunately there were no injuries from the two incidents, but mining was suspended at both operations last week.

The Daily Mercury quoted Queensland chief inspector of coal mines Gavin Taylor as saying that inspectors had suspended operations because of “ineffective safety and health management systems”

The mines would remain suspended until inspectors were satisfied, he reportedly said.

AAMC is investigating both incidents.
