The Fenzy Biocel 90 SCSR was past its use-by date when a miner tried to activate the unit by extending the breathing bag.
A plug within the bag failed to dislodge from one side of the canister, causing a blockage in one side of the breathing circuit.
The plug dislodged when the pressurised exhalation side of the breathing bag was squeezed so the miner donned the SCSR and used it to evacuate the mine.
At the mine’s surface, the breathing bag of the SCSR was cut open to identify the cause of the malfunction, and dust was found at the bottom of the breathing bag.
The Mines Inspectorate reminded all mines using this type of SCSR to activate the units and make sure they were operating correctly.
It called for any malfunctions or issues to be brought to the attention of the manufacturer, supplier of the SCSR and the Mines Inspectorate.
