Fletcher issues warning in accident's wake

Initial information indicated the roof bolter operator (using a Fletcher DDO roof-bolting machine) sustained his injuries when the cap holding the feed cylinder pin in place broke loose, causing the boom to fall. Unfortunately, the operator had his leg under the boom at the time.

To avoid similar accidents, Fletcher’s bulletin reminded owners and operators of arm feed machines (DDO/LTDO roof bolters only) about the intrinsic dangers posed by the machinery.

Most crucially, operators must be wary of the numerous roof bolter pinch point areas. Placing any body part in a pinch point area could result in serious injury, even death.

Fletcher urged pinch points be discussed at operations’ next safety meetings, reiterating the necessity of keeping a safe distance from all moving parts, checking warning tags, and maintaining all machinery guards. It also strongly recommended the use of OEM components, as well as regular inspections of roof-bolting machinery.

MSHA is still investigating the accident that occurred January 31 2005 in a West Virginia mine.
