A FATAL accident occurred at Hamilton County Coal’s Mine No 1 in Illinois because the mine operator failed to ensure the raised conveyor belt drive discharge boom had been securely blocked in position before miners performed work under it, a report by the US Mine Safety Health and Administration has found.
