A Rio Tinto spokesman said operations were ramping up as the WA Police finished their work at the site.

Operations were suspended on site in the wake of what Rio Tinto called a “serious incident”.

According to the WA Police, the deceased worker has been identified as David John Keen, 51 years old from Warnbro, in Perth’s south.

Police are preparing a report for the coroner.

The WA Department of Mines, Industry Regulation and Safety had no formal update on the incident, although it has an inspector from its Resources Safety division on site conducting further investigations.

The July 4 incident was the first mining fatality in WA in almost a year. The last mining fatality was on July 28 2016.
