Opal mine death foreseeable, report says

A NEW South Wales opal miner who was fatally struck by a 133kg super hoist bucket while working 13 metres underground at the Mulga Rush opal fields near Lightning Ridge in 2016 last underwent safety awareness training more than 18 years ago, according to a report by the NSW Resources Regulator.
