NINETEEN workers were forced to take shelter in emergency refuge chambers after a fire broke out on an agitator truck at an underground metal mine in Cobar, New South Wales on April 13.

According to the NSW Resources Regulator, the fire took hold of the dump truck when a punctured fuel tank caused fuel to spill as the truck was being driven down a decline.
