Fatality findings

WESTERN Australia’s Department of Mines, Industry Regulation and Safety has released its initial findings into the death of loader operator Michael Johnson who was fatally injured in July at Saracen’s Dervish underground mine when his load haul dump truck fell 25m over the edge of an open stope.

According to DMIRS' report Johnson was preparing to build a safety bund about 6m from the stope edge at about 1:45am when the vehicle fell through the void.
