A STUDY into a fatal accident involving wear plates from an excavator bucket at a Queensland open cut coal mine has found residual stress must be released in a controlled manner to maintain safety while removing large wear plates.

After the load was released, the welds carried a high residual tensile stress.

The fatal incident occurred in August 2017 at the BHP-Mitsubishi Alliance's Goonyella Riverside mine during maintenance works on the underside of the excavator bucket.
