AN INVESTIGATION into the death of Brian Bryant (70) at New South Wales’ Grawin opal fields in October 2020 found the opal miner died from neck and head injuries after falling 7m down a mine shaft.

Hoist machinery cage and perimeter fencing around the hoist shaft.

Bryant was working in an underground mine equipped with a powered material hoist that was installed in the main shaft, to haul opal dirt 7m to surface.
