Authorities pin Qld chem plant blast blame on failed butterfly valve

INCORRECTLY reinstalling the bearing bush bolts of a butterfly valve on hydrogen plant equipment caused a September 17 explosion and fire at a chemical processing plant in North West Queensland, the Mineral Mines and Quarries Inspectorate has found.

Three workers were injured, however, none needed hospitalisation. Two had been recommissioning the equipment and the third was a store worker who was about 40m away.
